Mason killed in fall from tubular-welded scaffold

Employee #1 fell approximately 28 1/2 ft while attempting to step from a tubular-welded frame scaffold onto the eave of a roof 27 to 40 in. from the edge of the scaffold. He sustained multiple fractures and was killed. The scaffold had been erected at the chimney of a two-story single family dwelling. Brick masons were not provided with safe means of access to the roof to complete a course of brick on the roof side of the chimney.
