Investigation Summary
On May 19 at 4:15 p.m., Employee #1 was standing on a wooden pallet that was approximately 25 feet in the air and positioned by a Lull, motorized rubber tire forklift. A coworker was driving the forklift. The men had been working about three to four hours installing precast concrete shapes on the exterior of a building. The machine had been repositioned along the building wall next to a creek. The ground under the machine was filled with debris--broken bricks and scrap wood. The lift's left rear wheel settled into the fill and, with the boom extended 30 feet at an elevated angle from horizontal of 60 degrees, the inertial momentum of the extended boom and the weight of Employee #1 and two concrete window sills that weighed approximately 50 lb each caused the Lull forklift to overturn. Seriously injured, Employee #1 was airlifted to UNC Memorial Hospital.
