Investigation Summary
At approximately 9:00 a.m. on May 2, 1989, Employee #1 was working alone on a 10-10 A-frame extension ladder. He had been instructed to remove and replace conduit that was run inside a concrete block wall. An opening for ductwork had been made and was large enough for a man to climb through. The bottom of the opening was 14 ft 3.5 in. above the floor. The conduit would have been accessible through the duct opening. No there were no witnesses; however, to remove the conduit (inside wall) Employee #1 would have been working at the 14 ft 3.5 in. location. The employee fell from the 10-10 A-frame extension ladder or from the wall opening onto concrete floor which resulted in severe head injuries. As of July 17, 1989, the employee was still hospitalized. Lack of employee training and failure to use personal protective equipment contributed to this accident.
