Investigation Summary
On October 20, 1990, several employees were installing a storm drain alongside a city street. Employee #1, while measuring the distance between two pipes in a 12 ft deep excavation, was knocked to the excavation floor when a 6 ft section of the north soil bank fell on him. He was pinned against the south bank and covered to his neck and shoulders. He sustained multiple bruises, but did not require hospitalization.
