Investigation Summary
Employee #1 was a member of a five-man crew installing a built-up roof. Four members of the crew had removed a skylight covering a roof hole so that the rubber membrane could be properly installed to seal the skylight. The rubber membrane was covering the opening. As Employee #1 walked past the opening he stepped to his left onto what he believed to be rubber membrane stretched over a steel deck. Employee #1 stepped onto the unsupported membrane and broke through. Employee #1 fell 28 ft to the concrete floor below, hitting a steel bar joist during the fall. Employee #1 was killed in the fall.
