Investigation Summary
Employees were laying felt atop a 20 ft 6 in. high, 12:12 pitched roof. Employee #1 was standing against a 2 by 4 toe board of broken lengths. The employee cut a section of that 2 by 4 toe board. This portion of toe board was not nailed down or otherwise secured. The toe board turned outward and the employee fell from the roof and struck a 2 ft thick concrete slab. He was killed.
