Investigation Summary
At approximately 9:30 on August 6, 1992, Employee #1 was working with a pipe crew laying 18 ft by 8 in. sewer pipe in a 12 to 13 ft trench. Employee #1 was standing at the east end of the trench, waiting for the pipe to be placed into position. When the pipe was laid at the bottom of the trench, he moved into the excavation to connect the pipe to one that already had been installed in the trench. A large portion of the north wall broke free and fell across the open area of the trench. Employee #1 was pinned against the south wall and died of crushing chest injuries.
