Employee dies after falling through roof opening

At approximately 11:30 a.m. on October 23, 1989, Employee #1 was bent over using an electric screw gun to fasten a sheet of metal decking on a roof. There was an opening measuring 15 ft by 75 ft where the old roof had been removed. The employee slipped at the edge of the metal sheeting and fell to the inside of the building 30 ft to a concrete floor. Employee #1 suffered multiple fractures and head injuries resulting in his death.
