Investigation Summary
At approximately 2:30 a.m. on September 4, 1992, Employee #1 was hooking up the return pipe for the filler station CIP (cleaning in place). The auger was running while Employee #1 attempted to connect the pipe. The pipe was approximately 1/8 inch too short and when Employee #1 finally got the return to meet the other pipe, the CIP cart moved away from the platform on which he was standing and he was thrown off balance. Employee #1 threw his hand out to catch himself and his right hand entered the filler opening. The tip of one of his fingers was amputated by the auger.
