Investigation Summary
On July 18, 1994, Employee #1, a yard jockey, was responsible for moving and parking vehicles in the company's L-shaped three-acre fleet parking area. Employee #1 was observed in different areas throughout the evening and was last seen approximately 5 minutes before he apparently came from a small access road and stepped into the path of a tractor trailer traveling at a slow rate of speed. Employee #1 was hit by the left front wheel and run over by the 2nd and 3rd axles. He suffered massive trauma to his entire body and died. There were no witnesses to the accident and the trailer operator stated that he only saw a "red flash" before impact.
