THE TRENCH WAS NOT SHORED OR SLOPED AND HEAVY MACHINERY WAS IN USE ON BOTH SIDES OF THE TRENCH. THE EAST SIDE COLLAPSED AND COMPLETELY BURIED EMPLOYEE #1.
