EMPLOYEE #1 WAS INSTALLING INSULATION OVER STEEL ROOF JOISTS PRIOR TO THE INSTALLATION OF ROOF PANELS. AFTER STAPLING THE INSULATION TOGETHER AT THE SEAMS, HE WALKED OVER THE INSULATION TO PICK UP A ROOF PANEL. HE FELL THROUGH THE INSULATION 53 FEET TO THE GROUND AND WAS KILLED.
