ELECTRIC SHOCK - CONTACT WITH LIVE PARTS THRU METAL TAPE

AN EMPLOYEE WAS REWIRING AN OVERHEAD BRIDGE CRANE. THE CRANE WAS BEING UPGRADED FROM PENDANT CONTROL TO RADIO CONTROL. THE EMPLOYEE WAS USING A METAL TAPE TO PULL CONDUCTORS THROUGH A CONDUIT CONTAINING EXISTING WIRING. THE EXISTING CONDUCTORS WERE ENERGIZED AT THE TIME. THE END OF THE METAL TAPE SNAGGED ONE OF THE ENERGIZED CONDUCTORS AND PIERCED ITS INSULATION. THE EMPLOYEE, WHO WAS PULLING ON THE TAPE WITH BOTH HANDS, WAS ELECTROCUTED.
