Investigation Summary
At approximately 7:00 a.m. on December 9, 1996, Employee #1 was operating a Delta Rockwell table saw without a guard. The saw blade was about 2 to 3 inches above the table. After making a cut, Employee #1 was removing the excess materials from the blade area when his left hand came in contact with the blade. His left index finger was amputated and his thumb was partly cut off. Employee #1 was taken to the hospital, where the thumb was successfully reattached. The finger could not be saved.
