Investigation Summary
Employee #1 was using a bull float to smooth out wet concrete floor. The garage basement wall was not completely cured but was backfilled with #57 gravel, which placed a lateral load on the wall. Employees #1, #2, and a coworker remained in the basement to finish the floor. They attempted to pour the garage floor and spread wet cement toward the garage basement wall. The existing lateral force plus additional weight from the wet concrete caused the wall to collapse. The wall remained intact but fell on top of Employee #1, killing him. Employee #2 was struck by loose concrete blocks.
