Investigation Summary

Investigation Nr: 924027
Event: 07/15/1996
Employee killed in fall from roof

The president of a company and Employee #1 were measuring a roof with a 300 foot tape measure. The president was standing at an expansion joint a few feet east and south of the roof area that was just above the front entrance. He was holding the tape measure reel. Employee #1 took the end of the tape measure and walked west for 300 feet, until the tape measure stopped. The president and Employee #1 then started moving westward together. Employee #1 took the end of the tape to measure the roof's final 60 foot distance. The president was looking down at the tape to see the measurement, and then started walking west with his head still down to ensure that the tape reeled up correctly. When he looked up, he did not see Employee #1 on the roof. He ran down to the northwestern side of the building and saw Employee #1 on the ground, just west of a dumpster that contained flattened cardboard boxes. The president went down to his truck and called emergency services from his cellular phone. Employee #1 was pronounced dead at the University Medical Center in Jacksonville, FL at approximately 4:30 p.m.
