Employee killed in fall from mobile scaffold

Employee #1 and two coworkers were erecting a scaffold system by installing platforms between two mobile scaffolds. Three platforms had been installed and the employees were installing another. Platform installation consisted of pulling the platform from one mobile scaffold to the other by walking across a lower platform that had previously been installed. One employee was walking the platform across and he pulled it too far, causing the platform to fall off the scaffold tower. It fell to the platform below, striking Employee #1 on the head and knocking him off the platform. He fell approximately 50 feet to the concrete driveway and was killed.
