Investigation Summary
Employee #1 was the only operator of a single face corrugator, which is run only once a month to establish stock material to be used for the next month. Normally three rolls of stock are run through the machine. Around 5:00 a.m., Employee #1 started the boiler, filled the hot glue tank, and started the glue tank heater. The first roll of stock ran out and a new roll was installed. Employee #1 and two coworkers then took a break, and when they returned, they started the second roll. The glue was not sticking, so they made some adjustments and started the second roll again. When the paper did not feed into the cut-off feed rollers, Employee #1 climbed up on the work platform perch. The corrugating mechanical end of the corrugator had stopped but the rollers continued to run. Employee #1 pulled the single face corrugated stock with his right hand as he attempted to feed the end of the stock into the feed rollers. While pulling on the stock, his left foot slipped out from under him. When he tried to catch himself with his left hand, his hand slipped into the cut-off feed rollers. The top roller is free wheeling and the bottom roller is driven. Employee #1 yelled for someone to stop the machine, but by the time the machine was stopped, his left hand and half of his forearm had been pulled into the rollers. During an interview with Employee #1, he revealed he had always fed the stock into the rollers while the machine was running because he did not know of another way. He also stated that he did not know if there was a way to feed stock into the rollers without the rollers running.
