Investigation Summary
At 2:15 p.m. on October 18, 1994, at a field location in Coudersport, PA, a crew was installing a new 4 in. plastic pipe to tie into a main gas pipeline. A pincher tool was being used to squeeze off and stop gas flow on an existing plastic pipeline in order to hook up the new pipe. The pinching tool failed, causing gas to escape and ignite. Employees #1 through #5 were burned in the resulting flash fire.
