Employee killed in fall from roof

Employee #1, a roofer, was utilizing stacks of roofing material to access another roof at a higher elevation. When he stepped on an unstable part of the stored material, he lost his balance and fell approximately 10 ft 8 in. onto a concrete shipping dock. Employee #1 suffered multiple injuries to his head and chest, and was killed. Causal factors of this accident include improper storage of materials at the edge of the roof and the employee's decision to use this material as a short cut instead of using the access ladder provided.
