Investigation Summary
Employees were attempting to remove a fallen tree from a roof of a house and dormer. Employee #1 working on the lower side of the tree cut the tree which caused him and the cut tree part to slip down the roof. Employee #1 fell 10 feet to the ground and the tree cut part fell on top of him, fatally puncturing him.
