EMPLOYEE #1 LOOKED OVER THE GUARD AND DOWN AT THE CHAIN DRIVE AT THE SAME TIME AS THE MAIN FRAME AND SLIDER DECK WERE MOVING DOWNWARD TO THE HOME POSITION. HE WAS PINNED BETWEEN THE MAIN FRAME AND THE GUARD AND WAS KILLED.
