EMPLOYEE WAS USING A REVERSE TILLER HYDRAULIC FREIGHT ELEVATOR WITHOUT A CAR DOOR, AND WITH THE HOISTWAY DOORS OPEN. HE WAS NOT A TRAINED OPERATOR. HE BECAME CAUGHT BETWEEN THE CAR AND THE HOISTWAY OPENING, SUSTAINING BLUNT TRAUMA TO THE CHEST AND ABDOMEN.
