Investigation Summary

Investigation Nr: 165874.015
Event: 04/18/2024
Employee is found unresponsive, later dies of unknown causes

At 11:58 a.m. on April 18, 2024, an employee working for a structural concrete contractor was patching panels. The employee had arrived at work at approximately 9:30 a.m. At 10:30 a.m., the employee was seen on surveillance video ascending an aerial lift. At 11:58 a.m., a coworker (foreman) heard the lift alarm and went to investigate. The coworker found the aerial lift raised to approximately 25 to 30 feet and the employee unresponsive in the lift. The coworker lowered life, called 911 and began CPR. The employee was transported to Osceola Regional Hospital by the local fire and rescue department. But the employee died from unknown causes, likely due to an underlying medical issue.
