ON 3-14-85 AT ABOUT 2:20 PM EMPLOYEE #1 WAS REMOVING NUTS FROM CONNECTING BOLTS ON THE LOWER 25' SECTION OF A LEG OF A H FRAME STEEL POWER TRANSMISSION TOWER THEY WERE PREPARING TO ADD 10' EXTENSIONS TO THE TOWER LEGS HE HAD NEGLECTED TO SECURE THIS LOWER SECTION TO THE LINE TRUCK WHEN HE REMOVED THE LAST NUT THE SECTION WHICH HE WAS TIED TO BY HIS SAFETY BELT AND LANYARD FELL TO THE GROUND HE RECEIVED THE FOLLOWING INJURIES RUPTED HEART AND LUNGS CRUSHED CHEST AND BROKEN LEFT FEMUR HE WAS RPONUNCED DEAD AT THE HOSPITAL
