Employee is killed in fall from roof

Employee #1 and two coworkers, who worked for an industrial contracting company, were replacing an old roof with a new one. While they worked on one section of the roof, a gap that was approximately 16 in. wide by 45 ft long was created and only one section was protected by a piece of 4 ft by 8 ft plywood. Employee #1 walked through the unprotected section and fell a little more than 25 ft to the floor. He sustained multiple injuries and died.
