EMPLOYEE STRUCK ON HEAD BY FALLING BALE OF WOOD PULP

ON NOVEMBER 23, 1986, EMPLOYEE #1 WAS OPERATING A FORKLIFT TRUCK. HE HAD 5 BALES OF WOOD PULP ON THE TRUCK, AND WAS ATTEMPTING TO PUSH THEM FURTHER UP ON THE FORKLIFT. HE PUSHED THE LOAD AGAINST THE BOTTOM OF ROWS 4 AND 5, CAUSING 8 BALES TO FALL FROM THE TOP OF ROW 4. EMPLOYEE #1 WAS LEANING OUT THE LEFT SIDE OF THE TRUCK AND WAS STRUCK ON THE RIGHT SIDE OF HIS HEAD BY ONE OR MORE OF THE FALLING BALES. HE DIED LATER THE SAME DAY FROM A SKULL FRACTURE AND CEREBRAL TRAUMA.
