Event: 09/21/2022
Employee is killed in fall from scaffold

At 3:15 p.m. on September 21, 2022, Employee #1 (masonry worker) was installing 16 inch by 8 inch by 8 inch concrete blocks from a fabricated frame scaffold equipped with bracket-supported working platforms on the side of the scaffold nearest the block wall. Employee #1 was standing on a bracket-supported working platform decked by two (9 inch wide by 11 feet long) wooden boards. This working platform was located approximately two feet above the first buck decking and less than 10 feet above the ground level. At around 3:20 p.m., Employee #1 reportedly fell backward onto the first buck decking. Employee #1 arrived at the hospital at around 5:00 p.m. Within 50 minutes of arrival, while undergoing CT imaging, Employee 1 became unresponsive and died at 6:10 p.m. The autopsy identified the cause of death as "multiple blunt force trauma due to fall from height."
