Investigation Summary

At 9:30 a.m. on April 29, 2023, an employee working as a process technician for an oil refinery was in a company break room. The employee did not look well resulting in emergency services to responded, measured the employee's vitals and cleared him to return to work. At approximately 11:30 p.m., the employee was conducting his rounds in Unit #1607. On April 30, 2023, at approximately 12:00 a.m., the employee was heard on the radio. At approximately 3:45 p.m., after the new shift had come on duty, coworkers found the employee in the break room unconscious. A coworker (shift supervisor) began CPR and external EMS was called. EMS services arrived and declared the employee dead at the scene. The cause of death was unknown at the time the narrative was submitted but was later determined to likely have been a heart attack.
