Investigation Summary
Employee #1 climbed into the top of a two-story shavings bin to retrieve a hard hat that had been sucked through a ventilation pipe. An overhead auger was automatically activated, presumably by Employee #1's movement inside the bin. His head became caught by the auger, and he was killed.
