Investigation Summary
A forklift driver was moving drums of toluene diisocyanate when he punctured one drum with his forklift blade. He removed the drum from the pallet and placed it where no more of the chemical would spill. He then took the pallet onto which toluene diisocyanate had spilled to the ship, where it was placed in the ship's hold so that other drums could be removed from the pallet. Employees #1 through #10 were exposed to the toluene diisocyanate in a variety of ways: Some were exposed from contaminated pallet boards, liquid on the drum, and liquid released in the actual spill; clerks were exposed while attempting to identify the spilled material; longshoremen were exposed to liquid from the drums; and the forklift driver was exposed by handling the drum. All 10 employees were hospitalized.
