AN EMPLOYEE WAS REMOVING THE COVER FROM AN ENERGIZED 4160-VOLT CONTROL TRANSFORMER. AS HE WAS DOING THIS, HE CONTACTED THE EXPOSED TERMINALS AND WAS ELECTROCUTED.
