Investigation Summary

Investigation Nr: 148177.015
Event: 07/25/2022
Employee dies due to natural causes

On July 25, 2022, an employee was on line 222 removing water condensation from the poly cool unit. The employee was in the area for approximately an hour and a half filling five pound buckets of water, while a coworker carried the filled water bucket outside. The employee left the area and found a second five pound bucket, which he subsequently filled and carried downstairs. He then informed another coworker that he was going to his office to cool down. The employee entered his office at 10:08 a.m. A coworker, who was passing by, noted that the employee was laying on the floor of the office at 10:24 a.m. The coworker then drove his powered lift to another coworker who was CPR-certified. The CPR-certified coworker called the incident out over the radio and proceeded to the area. Coworkers were able to unlock and enter the room at 10:26 a.m. They found the employee lying face down, under the desk, with his right hand pinned under his body. His body was in a "hunched" position. Coworkers attempted to rouse the employee, then pulled him out from under the desk and flipped him over. At 10:27 a.m. they began CPR, attached an AED, and called 911. Coworkers noted that the employee had a laceration and bruising on his forehead, as if he had struck it. There were no obvious fall hazards in the room. EMS arrived on scene at 10:35 a.m., attempted to resuscitate the victim, and transported him to Ascension Saint Thomas Hospital West, where he was pronounced dead. He died due to acute myocardial infarct due to thrombosis of left anterior descending coronary artery due to atherosclerotic and hypertensive cardiovascular disease. He died of natural causes.
