THROWN FROM OVERTURNING PRESSURE VESSEL

EMPLOYEE #1 WAS WORKING INSIDE A 12 FOOT 6 INCH DIAMETER, 13 FOOT LONG METAL PRESSURE VESSEL. HE WAS MARKING THE INSIDE OF THE VESSEL TO INDICATE WHERE TUBING WAS TO BE INSTALLED. EMPLOYEE #1 HAD A PENDANT CONTROL TO MANIPULATE THE ROLLERS UPON WHICH THE VESSEL WAS LYING. HE WAS ROLLING THE VESSEL AS HE WORKED. THE VESSEL WAS VERY HEAVY AT ONE END BECAUSE A HEAD HAD BEEN INSTALLED. AS A RESULT OF THIS WEIGHT IMBALANCE, THE VESSEL MOVED TOWARD THE HEAVY END WHEN IT WAS ROLLED. WHEN THE VESSEL ROLLED PAST ITS CENTER OF GRAVITY, THE OPEN END ROSE AND THE VESSEL FELL OFF OF ITS ROLLERS. EMPLOYEE #1, WHO HAD CLIMBED TO THE TOP OF THE OPEN END, WAS THROWN FROM THE PRESSURE VESSEL AGAINST AN ADJACENT VESSEL. HE DIED FROM THE SEVERE INJURIES RECEIVED TO THE BACK OF HIS HEAD.
