Investigation Summary
An employee had been working as a truck driver delivering scrap steel to a steel mill. On July 26, 2021, the employee may have been exposed to calcium oxide or lime dust while completing a delivery. The employee reported to a clinic on July 27, 2021, with symptoms that he believed were caused by chemical inhalation and presented with a chemical rash on his skin. He had reported the alleged exposure to his employer that same day. The clinic sent the employee to the hospital for follow up and he was admitted on August 1, 2021. While at the hospital, the employee was tested and found to be positive for COVID-19. On August 20, 2021, the employee died, and the hospital ruled the cause of death COVID-19. However, the coroner's office is questioning relatedness to chemical exposure and the cause of death is pending a pathologist's histology report, which was received January 6, 2022. Follow up is required in the future to determine the exact cause of death. The employer did not have the required Accident Prevention Plan (APP) which would have outlined the required prevention policies and procedures for protection against COVID-19.
