Investigation Summary
An employee had been working with patients in a skilled nursing facility. The employee's job title was not indicated in the narrative, but on or about August 17, 2021 the employee had worked with COVID-19 positive patient. On August 21, 2021 the employee tested positive for COVID-19 and quarantined at home. On August 29, 2021, the employee was admitted to the hospital. On October 5, 2021, the employee died. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative. The employer did not have an adequate respiratory protection program in place.
