Employee with COVID-19 infection dies

At 6:00 a.m. on November 29, 2021, an employee with COVID-19 infection died. The employee had worked on a construction jobsite as a production coordinator where a COVID-19 outbreak had occured affecting at least 5 coworkers. Coworker #1 last worked on the jobsite on November 19, 2021. On November 22, 2021, Coworker #1 started having COVID-19 related symptoms and other workers who were in close contact in Coworker #1's department started having symptoms too. Coworker #1 tested positive on November 21, 2021. Coworker #2 last worked on the jobsite on November 23, 2021, and tested positive on November 24, 2021. Coworker #3 last worked on the jobsite on November 23, 2021, and tested on November 23, 2021, and was positive. Coworker #4 last worked on the jobsite on November 28, 2021, and tested November 28, 2021, and was positive. Coworker #5 last worked on the jobsite on November 28, 2021, and tested on November 26, 2021, and was positive. The employer did not have an effective COVID-19 Prevention Program in place.
