Investigation Summary
On December 3, 2021, an employee who had been working as a full-time school teacher at at an education center teaching onsite classes died from COVID-19 infection. The employee began to experience COVID-19 related symptoms on November 5, 2021, and tested positive for COVID-19 on November 12, 2021. On November 15, 2021, the employee was hospitalized. The employer stated there was a COVID-19 outbreak at the school which caused the school to close and conduct classes virtually from November 11, 2021 until November 29, 2021.
