Investigation Summary

Investigation Nr: 141939.015
Event: 12/13/2021
Employee is killed in motor vehicle fire

At 11:15 p.m. on December 13, an employee was sitting in the driver's seat of her personal vehicle with the engine running. The vehicle gear was placed in reverse, causing the vehicle to move backwards, strike an island curb, and then strike a light pole. The wheels continued to spin in reverse while lodged against the pole. Friction heat was generated as a result. At some point, the vehicle caught fire. Heavy smoke and fire spread to the passenger compartment. The employee was killed due to burns and smoke inhalation.
