Investigation Summary
At 6:00 a.m. on November 24, 2021, an employee was troubleshooting an error code. The employee used the right side access door, when the hotplate welder cycled, and the hotplate traveled forward striking and pinning the employee. The employee was killed due to severe trauma to the head.
