Investigation Summary
An employee was working as a nurse at a nursing care or skilled nursing facility. The facility conducted random testing for COVID-19 twice a week. The employee tested positive for COVID-19 on November 4, 2021. No other employee tested positive, but nine residents did. The employee was quarantined. She was eventually admitted to the hospital a couple of days after testing positive. She died in the hospital on November 15, 2021.
