Employee with COVID-19 infection dies

On Monday, September 13, 2021, an employee had just returned to work as a Substance Abuse Counselor at a mental health and substance abuse facility after a getting over a stomach flu. On Friday, September 17, 2021, the employee started to experience COVID-19 related symptoms. On Saturday, September 18, 2021, the employee began to feel much worse and she quarantined herself at home. On Sunday, September 19, 2021, she took a COVID-19 test and tested positive. On September 26, 2021, the employee was hospitalized. At 4:00 a.m. on October 11, 2021, the employee died. There was a report that the employee's oxygen mask came off while in the hospital and was not immediately noticed. The hospital staff tried to revive her but were unsuccessful. The OSHA 300 Log indicated that three coworkers had COVID-19 with time off between September 8, 2021, and September 19, 2021, including the employee. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative.
