Investigation Summary
An employee had been working as a Registered Nurse for a Medical Center. The employee's last day of work was August 3, 2021. The employee began to experience symptoms August 6, 2021, and tested positive for COVID-19 on August 9, 2021. The employee was hospitalized on August 13, 2021 with COVID-19 related pneumonia and comorbidities to include asthma and emphysema. She had been placed in the ICU and was intubated for an extended period but she deteriorated and died at 11:15 p.m. on September 9, 2021. There was an outbreak of COVID-19 cases at her workplace and she may have contracted the disease there, although it has not been confirmed. The Employer did not provide an adequate record of contact tracing. For this reason, the employer was found to not have an effective COVID-19 Prevention Program in place.
