Investigation Summary
At approximately 3:00 p.m. on October 4, 2021, an employee last worked at the facility stripping wallpaper, painting, and fixing handrails. The wing in which the employee worked, housed COVID-19, dementia patients. Prior to October 4, 2021, the employee was working on projects throughout the building including putting up zipper walls outside COVID-19 positive resident's rooms, fixing beds, and fixing laundry machines. The employee tested positive for COVID-19 on October 5, 2021, and was admitted to the hospital on October 14, 2021. The employee died from complications from the virus on October 28, 2021.
