Investigation Summary
Employees were working at a Veteran's Administration hospital where there was a shortage of surgical masks and respirators. One employee contracted COVID-19 and subsequently died on October 9, 2021. A second employee contracted COVID-19 and subsequently died on December 1, 2021. During the time period the hospital had a COVID-19 cluster where five employees were confirmed to be infected with COVID-19.
