At 9:00 a.m. on September 17, 2021, an employee who worked as a direct service person potentially contracted COVID-19 from a resident who tested positive. On or about October 5, 2021, the employer reported that the employee was hospitalized, but later learned that the employee was actually treated and released from the hospital. At the time the employee contracted COVID-19, four coworkers also tested positive for COVID-19. In addition, two residents tested positive for COVID-19. Information regarding engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative.
