Investigation Summary

Investigation Nr: 139866.015
Event: 10/03/2021
Employee with COVID-19 infection dies

An employee was working as a nursing aide, orderly, or attendant at a continuing care retirement community. She worked as a resident assistant. She provided care and assistance to assisted living residents living in the retirement community. She provided residents with help doing their daily life skills, such as dressing, bathing, and mobility. On September 14, 2021, she began to feel tired and sluggish during her work shift. She believed it to be a side effect from her having received a flu shot. She called in sick on the following day, September 15, and she saw a doctor on September 21. At that time, she tested positive for COVID-19. On or about September 27, when her symptoms were not improving, she was admitted to the hospital. She died while in the intensive care unit (ICU), at 6:51 a.m. on October 3, 2021. There were no known COVID-19 cases in the facility among staff or residents before the employee became sick. The employee was believed to wear two procedure masks while in the facility assisting residents. Residents were not required to wear face coverings in the facility, and visitors were only required to wear face coverings while in common areas of the facility. Staff members were required to self-screen for signs and symptoms of COVID-19 prior to each shift in addition to twice-weekly COVID-19 testing for unvaccinated staff.
