Investigation Summary

Investigation Nr: 139762.015
Event: 08/30/2021
Employee with COVID-19 infection dies

On August 30, 2021, a coworker called an employer helpline to report he had potentially exposed an employee to COVID-19. The employee was a Carpenter for a school district and who had been working on a lock changing project with the coworker (also a Carpenter). The employee and coworker ate lunch and rode in a work truck together from an elementary school to a facilities building where they picked up needed parts and back to the school. The ride was approximately 25 minutes one way. On the evening of August 30th, the coworker called the employer helpline to report he was not feeling well and listed the employee as a contact in the event it was a COVID-19 related illness. The helpline called the employee and told him to quarantine. The employee began to develop COVID-19 related symptoms on September 2, 2021, and tested positive for COVID-19 on September 7, 2021. The employee was hospitalized on September 10, 2021, and died on September 24, 2021. Both the employee and the coworker had their own work truck and gas supplied by the school system. Masks are optional, but employees had been supplied with reusable masks. Schools are sanitized daily. Information regarding other engineering, administrative and personal protective equipment controls for COVID-19 are not included in the narrative.
