Investigation Summary

An employee was working as a nurse practitioner in an occupational health clinic where she saw patients for occupational health issues and procedures or for follow-ups from previous appointments. The employee developed symptoms of COVID-19 on September 21, 2021, and called in sick to work. On the same day, the employee tested positive for COVID-19 and was medically removed from work. The employee visited the hospital emergency room for care but was not admitted. The employee returned to her home using supplemental oxygen until again returning to the hospital emergency room. The employee was admitted the the hospital and died at 2:51 p.m. on September 26, 2021. In addition to the employee, two coworkers at the occupational health clinic developed symptoms, tested positive for COVID-19, and were medically removed from work. Both recovered from their infections.
