Investigation Summary

Investigation Nr: 139834.015
Event: 09/20/2021
Employee with COVID-19 infection dies

On September 5, 2021, Employee #1 did not appear for his night shift and a welfare check was initiated since he had pre-existing health issues that required wellness checks. A coworker was working night shifts as a nurse looking after mental health patients when he was sent home sick on August 29, 2021. The coworker was fully vaccinated against COVID-19 but his test came back positive on September 3, 2021. The employer then initiated testing of all hospital staff who had worked with the employee. At 7:17 a.m. on September 20, 2021, Employee #1 was discovered dead due to cardiac arrest from shock as a result of his pre-existing condition of diabetes. A coworker was discovered to have died with COVID-19 in his system however, it was the professional opinion of the funeral home director that the underlying cause of death was not COVID-19 and was due to natural causes.
